VIRTUAL PROGRAMS CONFIDENTIALITY & SAFETY

Our staff and volunteers respect the privacy of our members. The Cancer Support Community of Greater Ann Arbor (CSC) adheres to professional and ethical standards of confidentiality established by professional organizations and state law. Legal and ethical exceptions to confidentiality include: a clear or present danger to harm oneself or another, knowledge of the abuse or neglect of a minor child or incapacitated adult, or responses to a court subpoena or as otherwise required by law.

We ask that our members respect each other’s privacy and do not disclose the identity of others who participate at CSC, or what is said in a support group or program. However, we are not able to ensure the confidentiality of all members, as we would have no way to enforce it.

By participating in CSC’s virtual programs, I acknowledge that:

– I understand that CSC cannot ensure confidentiality.

– I understand that CSC reserves the right, at its sole discretion, to refuse or discontinue the use of its programs to any person.

– I understand that CSC does not provide medical advice or therapy or assistance of any kind.

– I understand that CSC’s virtual program does not include therapy or medical care.

In consideration of my acceptance as a member of The Cancer Support Community of Greater Ann Arbor (CSC), and the use of the programs and services provided without charge, I agree that I will not take any legal action against the CSC, its officers, agents, employees or any other participant or support person at CSC, based in any way on what is said or done by such officers, agents, employees and participants at CSC in a good faith effort to provide those services.

I fully understand that CSC recommends that anyone participating in any exercise program check with their healthcare provider prior to participation to ensure that they are medically able to do so. Participation in any exercise program could result in injury or illness.

By participating in CSC virtual programs, I agree to release, waive, discharge and hold harmless CSC and its employees, officers, agents, independent contractors, volunteers, board members, representatives, and donors from any and all claims, actions, demands, liabilities, expenses (including attorneys’ fees) and losses arising from any bodily injury or any other injury or illness, including, but not limited to, wrongful death, loss of services, loss of consortium, and all other damages that may arise out of my participation in any virtual program or the exercise program offered by CSC.

I also understand and agree that CSC is not actively marketing or offering its services in any area other than it’s designated service area within Michigan. If I live in a state other than Michigan, I acknowledge and agree that CSC has not engaged in any marketing or advertising in my home state or location, CSC has no contacts and does not do business within my home state or location, and that it is deemed that I have elected to use CSC’s services at CSC’s location within Michigan even if I am attending virtually.
